The following two forms must be completed if your company would like to respond to bids and quotes from Iowa State (Prison) Industries:

• Vendor Application (submit to ISI/IPI via mail, fax or e-mail)
• Substitute W-9/Vendor Update Form (submit to ISI/IPI via mail)
